Accessibility and Screen Readers

Can anyone provide me with any links to downloads, or at least share your experiences with screen readers?

I am currently working on improving the accessibility of my sites, and would like to get ahold of a screen reader to see (or rather, hear) what any vision-impaired visitors might come across when they visit my site. If I am understanding things correctly, the "title" and "alt" attributes are supposed to go along way toward helping my vision-impaired visitors understand what they can not see, but I'm not sure how screen readers interpret those sorts of things, and I want to make sure that I do things properly.
